About the role

We are seeking a full-time Compounding Pharmacist to join our long‑term care pharmacy in Lake Charles, Louisiana. The role involves ensuring safe, accurate medication therapy for residents of long‑term care facilities while complying with all regulatory standards.

Key responsibilities

• Review and verify medication orders for long‑term care residents to ensure accuracy and appropriateness.
• Conduct comprehensive drug regimen reviews and identify potential medication‑related issues.
• Collaborate with physicians, nurses, and healthcare teams to optimize patient medication therapy.
• Monitor for drug interactions, contraindications, and adverse effects.
• Ensure compliance with federal and state regulations governing long‑term care pharmacy operations.
• Provide consultant pharmacist services and recommendations to facility staff.
• Participate in interdisciplinary care planning meetings and provide clinical expertise.
• Maintain accurate documentation and records in accordance with regulatory requirements.
• Educate healthcare staff on proper medication administration and storage procedures.
• Respond to medication‑related inquiries and provide drug information support.
• Oversee medication storage, handling, and disposal protocols within the facility.

Required profile

• Pharm.D. degree.
• 1+ years of pharmacy experience preferred; new graduates are encouraged to apply.
• Active, in‑good‑standing Louisiana State Pharmacist License (or eligibility to obtain).
• Ability to successfully complete background, health, and drug screenings.
