Compassionate Home Health Occupational Therapist

About the role

BAYADA Home Health Care is seeking a full‑time Occupational Therapist to provide in‑home care for patients in the Philadelphia area. You will work directly with clients in their homes, helping them achieve safe, independent living after hospital discharge.

Key responsibilities

• Conduct comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Develop and continuously revise individualized intervention plans to improve independence and quality of life.
• Assess home environments for hazards, recommend equipment and adaptations, and educate patients and families on safe practices.
• Collaborate with nursing and multidisciplinary teams to integrate occupational therapy into overall care plans.

Required profile

• Active, current Pennsylvania license to practice as an Occupational Therapist.
• Minimum one year of recent occupational therapy experience.
• Ability to travel to client homes within the assigned geographic territory.
• Strong assessment, communication, and compassionate patient‑centered care skills.
