About the role

The Community Health Worker I will provide direct health support to residents of Craighead, CO, acting as a bridge between patients and clinical services. This position is part of the University of Arkansas for Medical Sciences’ effort to improve community health outcomes.

Key responsibilities

• Conduct health assessments and gather patient information in community settings.
• Provide health education, counseling, and resources to individuals and families.
• Coordinate referrals and follow‑up appointments with clinical teams.
• Maintain accurate documentation in compliance with privacy regulations.
• Support the OB‑GYN postpartum call center by responding to patient inquiries.

Required profile

• Legal authorization to work in the United States on the first day of employment.
• High school diploma or equivalent; additional health‑related training preferred.
• Strong communication skills and ability to work collaboratively with multidisciplinary teams.
