About the role
The University of Arkansas for Medical Sciences (UAMS) seeks a dedicated Community Health Worker (CHW) to strengthen connections between patients, community organizations, and the health system. The CHW will use a community‑informed approach to deliver education, resources, and support aimed at reducing health disparities across Arkansas.
Key responsibilities
- Develop and maintain relationships with patients, local community‑based organizations, and community members.
- Provide health education, resource navigation, and individualized goal planning.
- Coordinate outreach and engagement activities, especially for Level II and Level III CHWs.
- Plan, create, and deliver outreach materials and interventions using a community‑based participatory research framework.
- Participate in a multidisciplinary team to support case management, group education, and community support initiatives.
Required profile
- Demonstrated experience in community health, outreach, or public health settings.
- Ability to work collaboratively with diverse populations and organizations.
- Strong communication and interpersonal skills.
- Legal authorization to work in the United States on the first day of employment.
