Clinical Scheduler – Home Health

About the role

The Clinical Scheduler supports our home‑health clinicians by creating and maintaining accurate schedules that align with patient authorizations and needs. This onsite position works closely with the clinical team and administrative staff to ensure smooth operations and high‑quality patient care.

Key responsibilities

• Create, update, and maintain clinician schedules based on authorizations and patient requirements.
• Maintain an up‑to‑date list of clinician availability.
• Serve as the primary point of contact for scheduling‑related interactions with clinical and administrative team members.
• Track visit frequencies to guarantee proper scheduling compliance.
• Collaborate with the Director of Nursing to monitor clinician productivity.
• Assist with staff schedule changes and reassign patients as needed.
• Foster positive relationships with internal and external customers while upholding company core values.

Required skills

• 1+ year experience in home‑health healthcare scheduling.
• Fluent Spanish (preferred).
• Proficiency with Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Experience with Home Care Home Base EMR (a plus).
