Clinical Pharmacist – Medication Therapy Management

About the role

We are seeking a compassionate Clinical Pharmacist to join a leading healthcare organization focused on personalized medication management. In this role you will directly support patients by reviewing their medication regimens, ensuring safety and adherence, and providing clear guidance on proper use.

Key responsibilities

• Conduct outbound calls to patients and perform comprehensive medication reviews.
• Deliver in‑depth consultations on medication use following clinical guidelines.
• Accurately record prescription, diagnosis, and recommendation details in a web‑based application.
• Manage medication therapy services, offering guidance and support to improve patient outcomes.
• Assist in creating standardized patient documentation with clarity and accuracy.
• Participate in ongoing training to maintain and enhance clinical knowledge.

Required profile

• Valid Florida Pharmacist license (proof required).
• Minimum 4 years of experience in clinical medication therapy management.
• Strong understanding of pharmaceutical and medical terminology.
• Experience with quality measures related to medication use and patient safety.
• Fluent in English (spoken, written, reading) with excellent communication skills.
• Ability to maintain confidentiality and handle sensitive patient information.

Required skills

• Proficiency with customer relationship management (CRM) software.
