Clinical Pharmacist

About the role

Option Care Health is seeking a licensed Clinical Pharmacist to join its national home‑infusion team. This entry‑level position focuses on delivering safe, evidence‑based pharmacy services to patients receiving care in their homes across the United States.

Key responsibilities

• Assess patient eligibility for home‑care services by reviewing medical conditions, medication regimens, and prognosis.
• Develop, recommend, and monitor individualized pharmacy care plans for home‑infusion patients.
• Identify and resolve medication‑related problems, adjusting therapy or monitoring parameters as needed.
• Collaborate with nursing staff to coordinate patient services and track progress toward therapeutic goals.
• Provide education to patients, caregivers, and healthcare team members on appropriate medication use.
• Conduct nutritional assessments and incorporate findings into care plans.

Required profile

• Current license to practice pharmacy in the United States.
• Entry‑level clinical pharmacy experience or strong academic foundation in pharmacotherapy.
• Ability to evaluate patient data, make therapeutic recommendations, and document care plans.
• Excellent communication skills for patient and interdisciplinary team education.

Required skills

• Knowledge of home‑infusion pharmacy regulations and standards.
• Proficiency in medication therapy management and clinical documentation.
