Clinical Pharmacist

About the role

Option Care Health is seeking a licensed Clinical Pharmacist to join its home infusion team. This entry‑level position focuses on delivering safe, effective pharmacy services to patients receiving care at home across the United States.

Key responsibilities

• Determine patient suitability for home infusion based on medical condition, prescribed therapy, and outcome goals.
• Collect and organize patient‑specific information to identify medication‑related problems.
• Design, recommend, monitor, and evaluate individualized pharmacy care plans.
• Make evidence‑based recommendations for medication adjustments and monitoring parameters.
• Perform nutritional assessments and incorporate findings into care plans.
• Modify pharmacy care plans as monitoring data and patient status evolve.
• Coordinate services with the Nursing Department and educate patients, caregivers, and staff on proper medication use.

Required profile

• Active pharmacist license (PharmD or equivalent) in the United States.
• Entry‑level clinical pharmacy experience or strong interest in home‑care pharmacy.
• Ability to assess patient suitability for home infusion and develop care plans.
• Strong communication skills for patient and interdisciplinary team education.
