About the role
We are seeking a Clinical Pharmacist to join our enterprise clinical programs, supporting provider and patient engagement, medication optimization, and drug strategy initiatives. The role works closely with medical leadership, care teams, and operational stakeholders to improve quality, outcomes, and cost of care in a value‑based environment.
Key responsibilities
- Operate within pharmacist‑led programs focused on chronic disease, quality improvement, and medication optimization.
- Identify eligible patients, conduct outreach, enrollment, and ongoing engagement through education and follow‑up.
- Perform medication reconciliation, medication therapy management (MTM), and comprehensive medication reviews (CMR) via telephone.
- Provide actionable medication recommendations and collaborate with providers using technology and direct communication.
- Support other clinical programs with pharmacy‑related consultation and drive medication adherence to close quality gaps.
- Collaborate with interdisciplinary teams (physicians, health coaches, nurses) to develop patient‑specific action plans and adjust therapies.
- Accelerate adoption of clinical insights by delivering drug‑related recommendations and feedback to improve decision‑support tools.
- Partner with regional medical officers and practice transformation managers to ensure consistent clinical standards and workflows.
- Contribute to specialty medication, high‑cost drug, and pharmacy optimization initiatives aligned with value‑based care goals.
- Document all clinical activities in compliance with state, federal, and HIPAA regulations and serve as a pharmacy information resource.
Required profile
- Doctor of Pharmacy (PharmD) degree.
- Active pharmacist license in good standing in one of the listed states (CA, WA, KS, OH, NY, NJ, FL, MD, NH, MS, LA).
- Experience in clinical pharmacy, medication management, and patient engagement.
- Ability to work cross‑functionally and translate data‑driven insights into clinical impact.
- Knowledge of clinical guidelines, quality measures, and pharmacy best practices.
