Clinical Pharmacist

About the role

Option Care Health, the nation’s largest home infusion provider, is seeking a Clinical Pharmacist to join its growing team. This entry‑level position focuses on delivering safe, evidence‑based pharmacy services to patients receiving care in their homes across the United States.

Key responsibilities

• Assess patient eligibility for home infusion services based on medical condition, prescribed therapy and prognosis.
• Collect and organize patient‑specific information to identify medication‑related problems.
• Design, recommend, monitor and evaluate individualized pharmacy care plans for home‑care patients.
• Modify therapy recommendations and monitoring parameters as needed to achieve defined outcome goals.
• Coordinate patient services with the Nursing Department and other clinical staff.
• Educate patients, caregivers and healthcare professionals on appropriate medication use.
• Perform nutritional assessments and incorporate findings into care plans.

Required profile

• Active pharmacist license in the United States.
• Entry‑level clinical pharmacy experience or strong interest in home‑care pharmacy.
• Ability to work collaboratively with multidisciplinary teams.
• Strong analytical and communication skills.
