Clinical Pharmacist

About the role

The Clinical Pharmacist joins Option Care Health, the nation’s largest independent home and alternate site infusion provider. Working Monday‑Friday on day shift with on‑call duties, the pharmacist ensures safe, effective medication therapy for patients receiving care in their homes across the United States.

Key responsibilities

• Determine patient suitability for home infusion based on medical condition, prescribed therapy, and outcome goals.
• Collect and organize patient‑specific data to identify and resolve medication‑related problems.
• Design, recommend, monitor, and evaluate individualized pharmacy care plans.
• Make evidence‑based recommendations for medication adjustments or monitoring parameters.
• Coordinate care with the Nursing Department and educate patients, caregivers, and staff on proper medication use.
• Perform nutritional assessments and incorporate findings into therapy plans.

Required profile

• Current, active pharmacy license in the United States.
• Entry‑level clinical pharmacy experience or a strong interest in home‑care pharmacy practice.
• Ability to assess patient eligibility for home infusion services according to Option Care policies.
• Strong communication skills for interdisciplinary collaboration and patient education.

Required skills

• Knowledge of pharmacy practice standards and regulatory requirements.
• Proficiency in medication therapy management and pharmacotherapy planning.
• Ability to conduct nutritional assessments related to medication therapy.
