Clinical Pharmacist

About the role

We are seeking a Clinical Pharmacist to join our multidisciplinary team at UChicago Medicine AdventHealth in Hinsdale, Illinois. The pharmacist will provide safe, timely, and cost-effective medication therapy across inpatient, satellite, and IV pharmacy areas while supporting patient education and interdisciplinary care.

Key responsibilities

• Educate patients, families, and staff on safe and effective medication use.
• Participate in antimicrobial stewardship, surveillance, and restricted-use programs.
• Engage in direct patient care, interdisciplinary rounds, and transitions of care from admission to discharge.
• Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents.
• Identify and resolve medication safety events, adverse drug reactions, and errors.
• Utilize EHRs, pharmacy informatics, and medication automation to optimize clinical decision support.
• Perform compounding, dispensing, verification, and documentation of medications.
• Conduct pharmacist consults, pharmacokinetic dosing, monitoring, and adjustments.
• Support quality improvement, regulatory compliance, and accreditation activities.

• Electronic Health Record (EHR) proficiency.
• Pharmacy informatics system experience.
