Clinical Pharmacist – Home Infusion Services

About the role

The Clinical Pharmacist joins Option Care Health, the nation’s largest independent home infusion provider, to deliver safe and appropriate pharmacy services for patients receiving care at home. This entry‑level position focuses on assessing patient suitability, creating individualized pharmacy care plans, and collaborating with nursing and other clinical teams.

Key responsibilities

• Determine the suitability of patients for home infusion based on medical condition, prescribed therapy, and outcome goals.
• Collect and organize patient‑specific information to identify and resolve medication‑related problems.
• Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans.
• Make evidence‑based recommendations for medication therapy modifications and monitoring parameters.
• Conduct nutritional assessments and provide related medication recommendations.
• Coordinate care with the Nursing Department and educate patients, caregivers, and staff on appropriate medication use.

Required profile

• Licensed pharmacist in good standing with a valid pharmacy license.
• Entry‑level clinical pharmacy experience or strong academic foundation in pharmacy practice.
• Ability to assess patient health status and determine home care eligibility.
• Strong communication skills for patient and interdisciplinary team education.

Required skills

• Knowledge of pharmacy practice standards and regulatory requirements.
• Ability to interpret medication orders and develop care plans.
