Clinical Pharmacist – Home Infusion

About the role

The Clinical Pharmacist at Option Care Health evaluates patients for home infusion suitability, designs individualized pharmacy care plans, and ensures safe medication therapy across home and other care settings.

Key responsibilities

• Determine patient eligibility for home care by reviewing medical conditions, prescribed therapies, and prognosis.
• Collect and organize patient‑specific information to identify medication‑related problems.
• Design, recommend, monitor, and evaluate personalized pharmacy care plans for home infusion patients.
• Adjust medication regimens and monitoring parameters as needed based on patient data.
• Perform nutritional assessments and provide appropriate recommendations.
• Educate staff, patients, and caregivers on medication use and therapy goals.
• Maintain continuity of pharmaceutical care between home and other care settings through systematic communication.
