Clinical Nurse Coach (RN)

About the role

Renalogic is seeking a Clinical Nurse Coach (RN) to provide remote health coaching for members at risk for or living with chronic kidney disease. The role is fully remote within the United States, requires work on Pacific Standard Time hours, and may include occasional evenings, weekends, and travel for corporate events.

Key responsibilities

• Establish and maintain contact with members via video, phone, text, and email in English.
• Develop individualized care plans based on clinical information and NCQA guidelines.
• Assess members’ health status, coordinate care, and collaborate with their healthcare providers.
• Use motivational interviewing and the Triple Aim framework to empower members toward health goals.
• Document all interactions in compliance with HIPAA regulations and meet performance metrics for engagement and retention.

Required profile

• Active RN license in good standing with willingness to obtain licensure in additional states.
• Minimum five years of RN experience, preferably with chronic conditions or CKD.
• BSN preferred; CDE or CDCES certification strongly preferred.
• Ability to work PST hours, flex for evenings/weekends, and travel occasionally.
• Reliable internet, power, and a dedicated workspace.

Required skills

• Proficiency with Microsoft Office and remote communication tools.
• Experience with HIPAA‑compliant documentation and data handling.
• Motivational interviewing techniques.
• Knowledge of NCQA care‑plan guidelines.
