Clinical Documentation Specialist – Outpatient

About the role

The Clinical Documentation Specialist (Outpatient) at Reveleer will improve the quality and completeness of clinical documentation for outpatient records. This remote, full‑time position focuses on validating HCC diagnoses and ensuring accurate documentation to enhance risk adjustment outcomes.

Key responsibilities

• Review patient records to identify and validate Hierarchical Condition Category (HCC) diagnoses.
• Create provider queries and recommendations that reflect best practices and improve clinical workflows.
• Validate that documentation supports reported conditions and recapture suspect diagnoses.
• Maintain strict confidentiality of patient information in line with privacy regulations.

Required profile

• Graduate of an accredited nursing program with an active, unrestricted RN or Advanced Practice Provider license.
• Minimum 2 years outpatient CDI experience and 5 years of clinical experience.
• Certifications such as CDEO, CCDS‑O, CDIP, or CRC.
• Strong understanding of CMS, HHS, Medicaid, and Rx HCC models and current HCC documentation standards.

Required skills

• Proficiency with Epic, Athena, and Cerner electronic medical record systems.
• Knowledge of ICD‑10‑CM coding, AHA Coding Clinics, and Official Coding Guidelines.
