Clinical Coordinator – START Program

About the role

The START Clinical Coordinator works within the START (Systemic, Therapeutic, Assessment, Resources and Treatment) program to support individuals with intellectual or developmental disabilities and co‑occurring mental health needs. Employed by Community Family Advocates, the coordinator will operate onsite at Allegheny County DHS.

Key responsibilities

• Maintain an active caseload of clients referred for START services.
• Coordinate support meetings, crisis plans, and outreach visits to homes, day programs, and vocational settings.
• Develop comprehensive service evaluations, intake assessments, admission/discharge summaries, and cross‑system crisis prevention plans.
• Provide training to partner systems on START support plans and ensure timely data entry.
• Participate in daily triage calls, on‑call rotation, and the Center for START Services National Network.

Required profile

• Demonstrated competency in START coordination and systemic analysis using tools such as Ecomaps.
• Strong problem‑solving abilities and customer‑service orientation.
• Ability to work independently and collaborate with multidisciplinary teams.

Required skills

• Proficiency with Microsoft Word, Outlook, and Excel.
