Case Manager – Behavioral Health

About the role

Indiana Treatment Centers, in partnership with Ascension Recovery Services, is seeking a compassionate Case Manager to support individuals recovering from substance use and co‑occurring mental health conditions. You will work within a multidisciplinary team to coordinate care, remove barriers, and promote long‑term recovery.

Key responsibilities

• Provide comprehensive case management for clients with moderate to high clinical and psychosocial needs.
• Coordinate discharge planning and transitions of care to ensure continuity of services.
• Facilitate interdisciplinary case reviews, communicating progress, barriers, and recommendations.
• Assist clients in accessing housing, employment, transportation, insurance, benefits, and community resources.
• Build collaborative relationships with referral sources, probation officers, courts, employers, and community partners.
• Advocate for clients while encouraging accountability and self‑sufficiency.
• Maintain accurate, timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives, compliance activities, and team meetings.

Required profile

• High school diploma or equivalent; associate’s or bachelor’s degree in behavioral health or human services preferred.
• Minimum of one year of case management, care coordination, or direct service experience in behavioral health, substance use disorder treatment, mental health, or related settings.
• Knowledge of community resources and social service systems.
• Experience documenting in an Electronic Health Record (EHR).
• Current CPR/First Aid certification or ability to obtain within 30 days of hire.
