Care Coordination Assistant

About the role

The Care Coordination Assistant supports the SVT Health & Wellness clinical team in Homer, Alaska. This full‑time, on‑site role provides clerical and electronic records assistance to ensure smooth patient care coordination.

Key responsibilities

• Assist RN Care Coordinators with administrative tasks related to patient care.
• Perform medical office duties such as scanning, faxing, handling a multiline phone system, and message recording.
• Schedule appointments for case‑management patients.
• Maintain and retrieve health record indexes in both paper and electronic formats, ensuring compliance with SVT policy and legal requirements.
• Communicate with internal and external parties to request or release medical record information while safeguarding confidentiality.
• Process Release of Information requests and track electronic filings.
• Handle outside imaging orders, obtain prior authorizations, and monitor order status.
• Travel to nearby communities by small aircraft or boat as needed, with lodging and meal stipend provided for overnight stays.

Required profile

• Minimum 2 years of clerical experience, preferably in a clinical setting.
• Proficiency in medical terminology (preferred) or willingness to be trained.
• Associate’s degree or comparable education is a plus.
• Relevant certifications (RHIT, CHDA, CDIP, CEHRS) are preferred but not mandatory.

• Hourly wage of $22‑$24 based on experience.
