Alumni Coordinator

About the role

The Alumni Coordinator will design, launch, and sustain a comprehensive alumni program that supports patients as they transition into long‑term recovery. This role bridges discharged patients with ongoing recovery services, enhancing engagement, continuity of care, and reducing readmissions.

Key responsibilities

• Design, implement, and maintain a structured alumni program for discharged patients.
• Develop and manage an alumni engagement calendar, including groups, events, and outreach campaigns.
• Track alumni participation using CRM and/or EMR systems and maintain a consistent outreach cadence (e.g., 30‑, 60‑, 90‑day follow‑ups).
• Conduct post‑discharge follow‑up calls and facilitate both in‑person and virtual alumni support groups.
• Coordinate peer‑mentorship opportunities between alumni and current patients.
• Partner with clinical, operations, and business development teams to align after‑care planning with value‑based care initiatives.

• Detail‑oriented approach to maintain accurate alumni records and outreach schedules.
