Activities Coordinator

About the role

The Activities Coordinator supports the physical, cognitive, emotional, and social well‑being of participants enrolled in Habitat Health’s PACE program. Working within an interdisciplinary team, this role plans and delivers therapeutic recreation interventions that enhance functional ability, engagement, and quality of life for older adults with complex medical and psychosocial needs.

Key responsibilities

• Conduct comprehensive recreational and social assessments to evaluate functional abilities, interests, strengths, and needs, documenting findings per PACE regulatory standards.
• Design and lead therapeutic group activities that promote physical, cognitive, social, and emotional wellness.
• Create and maintain the center’s monthly activity calendar to engage all participants.
• Identify community resources that can support participants’ recreational and social needs.
• Coordinate with Physical and Occupational Therapists to integrate restorative goals into daily programming.
• Contribute assessment findings and SMART goals to interdisciplinary team care planning.
• Adapt activities to accommodate diverse functional levels, cultural backgrounds, and individual preferences.
• Monitor participant progress, update care plans, and document interventions and outcomes accurately and promptly.

Required profile

• Experience working with older adults who have complex medical and psychosocial needs.
• Ability to conduct assessments and develop individualized activity plans.
• Strong communication and teamwork skills within a multidisciplinary environment.
• Cultural sensitivity and flexibility to tailor programs to diverse participants.
