Virtual Support Assistant – Palliative Care at Home

About the role

Mount Sinai Morningside is looking for a Virtual Support Assistant to help coordinate home‑based palliative care. The role involves patient outreach, education, enrollment, consent handling and administrative support for clinical teams.

Key responsibilities

• Conduct patient outreach, provide program education, obtain consent, and coordinate enrollment scheduling.
• Support interdisciplinary care teams in addressing patient and family needs.
• Assist with admissions and ongoing home‑based patient‑care operations.
• Follow up on provider orders and coordinate with clinical teams and vendors.
• Track enrollments, appointments, discharge orders, and care activities.
• Communicate care plans and patient assessments to frontline healthcare teams.
• Participate in team meetings and care‑coordination discussions.
• Maintain accurate documentation as required.

Required profile

• High school diploma or equivalent; Associate Degree in Healthcare preferred.
• At least 1 year of healthcare experience.
• Experience with online ordering systems and data entry.
• Ability to work day and evening shifts, rotating holidays, and on‑call schedules.
• Strong organizational and communication skills.
• Experience with EMR systems preferred.
• Strong telephone communication and care‑coordination skills.
• Spanish‑speaking ability is an advantage.
