Social Worker (RSW) – Case Management – PRN

About the role

The Social Worker (RSW) provides generalist social work services within a medical setting on the Kings Highway campus. Working collaboratively with nurses, physicians, and allied health professionals, the role focuses on assessing patient needs, coordinating discharge plans, and linking families to community resources.

Key responsibilities

• Conduct patient assessment interviews and develop individualized discharge plans.
• Identify and provide appropriate community resources and referrals for patients and families.
• Maintain accurate patient records, document critical information, and communicate updates to the multidisciplinary team.
• Monitor resource utilization and ensure fiscally responsible delivery of services.

Required profile

• Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
• Minimum five years of full‑time social work experience post‑degree, preferably in a medical or community‑resource setting.
• Certified Social Worker (CSW) or Registered Social Worker (RSW) credential verified by LABSWE.
• Working knowledge of common disease entities, medical terminology, and eligibility criteria for Medicare, Medicaid, and commercial insurance plans.

Required skills

• Proficiency with Windows‑based applications and data‑entry software.
• Strong computer skills for retrieving and entering patient information.
• Familiarity with medical terminology and disease processes.
