Social Worker (Bone Marrow Transplant)

About the role

The Social Worker for Bone Marrow Transplant provides bio‑psychosocial assessments, patient education, and crisis interventions for patients and families. The role coordinates complex discharge plans and collaborates with a multidisciplinary team to address emotional, social, cultural, and spiritual needs.

Key responsibilities

• Conduct patient assessment interviews, provide referrals, and coordinate discharge plans.
• Identify and connect patients and families with community resources.
• Maintain accurate patient records and communicate critical information to the medical team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.
• Stay current with professional knowledge and required certifications through continuing education.

Required profile

• Master's degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); Licensed Master Social Worker (LMSW) preferred or obtainable within 6 months.
• No prior experience required; two years of medical‑setting experience preferred.
• Knowledge of health‑care systems, Medicare/Medicaid, and medical terminology.

Required skills

• Proficiency with Windows‑style applications and related software packages.
• Experience using electronic medical record systems.
