Social Services Program Consultant III

About the role

The Social Services Program Consultant III works within the Department of Community Supervision to develop, implement, and manage individualized reentry plans for persons under parole and probation supervision. This hybrid position collaborates with families, service providers, faith‑based groups, and other agencies to connect individuals with housing, employment, health care, and supportive services.

Key responsibilities

• Plan and execute individualized reentry strategies for supervised individuals.
• Coordinate case management activities and maintain accurate documentation.
• Engage and partner with community resources, including housing, employment, and health services.
• Facilitate communication between supervision caseloads, families, and volunteer organizations.
• Utilize technology platforms to track progress and outcomes.
