RN Case Manager – OMC Jefferson Highway (PRN)

About the role

We are seeking a Registered Nurse (RN) Case Manager to coordinate multidisciplinary care for patients throughout their hospital stay at Ochsner Health’s OMC Jefferson Highway location. The role involves reviewing utilization of resources, ensuring appropriate level of care, and facilitating communication among patients, families, clinicians, and payers.

Key responsibilities

• Perform pre‑admission, admission, and continued stay reviews according to CMS, commercial payer, and hospital guidelines.
• Maintain knowledge of participation conditions, utilization review, and discharge planning standards.
• Communicate with ED physicians, hospitalists, attending physicians, and physician advisors as a liaison.
• Coordinate and implement discharge plans, overseeing functional and financial outcomes during acute‑care hospitalization.
• Adapt communication style to respect patient privacy and individual needs.
• Maintain open communication with patients, caregivers, multidisciplinary team members, post‑acute providers, and third‑party payers.
• Support service excellence, teamwork, and integrity while complying with federal, state, and local regulations.
• Perform other related duties as required.

Required profile

• Registered nurse diploma or equivalent (required).
• Bachelor’s degree in nursing (preferred).
• Current RN license in the state of practice (required).
• Basic Life Support (BLS) certification (required).
• 0 years required experience; 3 years hospital‑based discharge planning, case management, or utilization review preferred.
• Case Management Certification (CCM) or Accredited Case Manager (ACM) preferred.
