RN Case Manager - Kings Hwy

About the role

The RN Case Manager provides multidisciplinary communication and facilitates care throughout a patient’s hospital stay. This role reviews utilization of resources, ensures appropriate level of care, and collaborates with patients, families, clinicians, and payers to develop discharge plans.

Key responsibilities

• Perform pre‑admission, admission, and ongoing stay reviews in accordance with CMS, commercial payer guidelines, and hospital policy.
• Serve as liaison between emergency department physicians, hospitalists, attending physicians, and physician advisors.
• Coordinate and implement discharge plans, addressing functional and financial outcomes.
• Document level‑of‑care orders and related documentation clearly in the medical record.

Required profile

• Registered Nurse license in Louisiana (required) with at least 3 years of hospital‑based experience.
• Bachelor’s degree in Nursing preferred; diploma or equivalent acceptable.
• Basic Life Support (BLS) certification required; Case Management certification preferred.

Required skills

• Proficient computer skills for data entry and retrieval of patient information.
• Ability to use Windows‑style applications and keyboard efficiently.
• Effective verbal and written communication for interacting with multidisciplinary teams.
