About the role

join9amHealth is seeking a compassionate Registered Nurse to provide virtual health coaching and chronic disease management for patients with diabetes, hypertension, hyperlipidemia, and obesity. Working entirely remotely, you will engage with members via chat, phone, and video to deliver personalized education and support.

Key responsibilities

• Deliver patient‑centric health coaching that aligns medical needs with individual preferences.
• Tri­age reported symptoms and side effects, escalating to providers when necessary.
• Provide comprehensive education on diabetes, hypertension, hyperlipidemia, and weight management through multiple digital channels.
• Coordinate care among patients, providers, support teams, and cross‑functional clinical members.
• Create consult documentation that follows internal protocols and standards.
• Accurately record patient interactions to meet documentation requirements.

Required profile

• Strong empathy, communication, and patient‑centered care abilities.
• Bachelor of Science in Nursing (BSN) required.
• Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed.
