Population Health Program Manager

About the role

This on‑site position reports to the Director of Practice Operations and leads the development, implementation, and performance management of population health initiatives that align with the organization’s strategy. The role works closely with clinical, operational and analytics leaders to drive care transformation and improve quality, outcomes and value across the continuum of care.

Key responsibilities

• Support strategic planning for population health programs, monitor key performance indicators and ensure best‑in‑class delivery.
• Partner with stakeholders to establish, integrate and oversee programs, including external payer collaborations.
• Coordinate with analytics to develop reporting tools and actionable reports.
• Act as a care‑transformation change agent, fostering strong physician partnerships and open communication.
• Supervise nurse navigators in outpatient clinics and perform additional duties to support departmental effectiveness.

Required profile

• Bachelor’s degree in Population Health, Clinical Quality/Process Improvement, or Nursing.
• Current, valid Arizona RN license.
• Minimum three years of experience in population health, quality improvement, nursing leadership or project management.
• Ability to walk up to 4 hours daily, sit up to 5 hours daily, and lift up to 25 lb.
