Planning List Administrator – Region 5 Field Office

About the role

The Planning List Administrator supports individuals and families who are eligible for NOW or COMP waivers across multiple counties in Georgia. Based in the Region 5 Field Office, the role begins with on‑site training and then transitions to remote work, providing critical coordination of Medicaid waiver services and community resources.

Key responsibilities

• Assist prioritized individuals with waiver service entry, completing required documentation and maintaining accurate records.
• Maintain up‑to‑date knowledge of local resources and refer families to non‑waiver supports as needed.
• Communicate with clients, families, and authorized parties to gather information on changing circumstances and support needs.
• Participate in regional planning list meetings to share information and improve team coordination.
• Guide families through Medicaid application processes and help individuals with IDD select service providers.
• Develop and sustain a network of community contacts to educate families and providers.
• Monitor health‑related needs of planning‑list members and facilitate access to Medicaid state‑plan supports.
• Ensure compliance with HIPAA and DBHDD confidentiality policies.
• Report any suspected abuse, neglect, or exploitation according to policy.

Required profile

• Experience working with Medicaid waiver programs or similar social‑service systems.
• Strong understanding of community resources and ability to build referral networks.
• Excellent communication skills, adaptable to the abilities of diverse clients.
• Commitment to confidentiality and adherence to HIPAA and state regulations.
• Willingness to travel up to 50 % across the state.
