Personal Care Attendant – Full‑Time

About the role

Habitat Health is seeking a compassionate Personal Care Attendant to support older adults enrolled in its PACE program. You will work primarily in the Sacramento center, with occasional visits to participants’ homes, helping them maintain independence and enjoy a high quality of life.

Key responsibilities

• Assist participants with activities of daily living, including feeding, ambulation, and personal hygiene.
• Provide timely, caring responses to participant requests and coordinate additional assistance when needed.
• Promote a welcoming, inclusive environment that encourages participants to spend time in the center.
• Collaborate with the care team to share feedback and improve clinical outcomes and participant experience.
• Uphold Habitat Health’s values and contribute to a positive team culture.

Required profile
