Part-Time Social Worker (MSW) – Case Management, 3 Days/week

About the role

The Social Worker (MSW) provides bio‑psychosocial assessments, patient education, and crisis interventions while coordinating complex discharge plans for patients at Ochsner Health.

Key responsibilities

• Conduct patient assessment interviews, provide referrals and coordinate discharge plans.
• Identify and provide helpful resources to patients and families.
• Maintain patient records and communicate critical information to the medical team.
• Participate in on‑call rotation and utilize management resources.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or to be obtained within 6 months.
• Knowledge of healthcare systems, Medicare/Medicaid, and medical terminology.
• Effective verbal and written communication and strong interpersonal skills.

Required skills

• Computer proficiency for data entry and Windows‑style applications.
• Ability to use electronic health record (EHR) systems.
