Palliative Care Nurse

About the role

Evergreen Family Medicine is seeking a compassionate Palliative Care Nurse to provide chronic care and case management services across its Roseburg clinic. The role focuses on patient‑centered care, symptom relief, and coordination with primary physicians and specialists.

Key responsibilities

• Maintain confidentiality in accordance with HIPAA and EFM policies.
• Adhere to departmental, state, and federal regulations.
• Communicate professionally with coworkers, managers, and patients via phone, email, or in person.
• Assess physical functioning, acute and chronic illness, and disability.
• Identify patient and family needs, develop and implement care goals, and coordinate resources.
• Monitor and document quality of care, ensuring care plans are understood and followed.
• Identify service delays and communicate them to the healthcare team.
• Visit patients in care facilities and provide chronic care management services.
• Manage symptoms such as pain, shortness of breath, fatigue, and depression.
• Facilitate discussions about patient goals, values, and care preferences.
• Coordinate comprehensive care plans with primary physicians and specialists.
• Provide emotional and psychosocial support to patients and families.
• Assist with advance care planning and legal directives.
• Connect patients and families to community services, support groups, and spiritual care.
• Perform other duties as assigned.

Required profile

• Valid LPN or RN license/certification.
• Strong communication, interpersonal, clerical, and organizational abilities.
• Ability to handle confidential information securely.
• Typing speed of at least 40 wpm and proficiency with computers and office equipment.
• Physical ability to sit for prolonged periods, walk frequently, lift up to 35 lb, and perform occasional stooping, kneeling, or crouching.
