Outpatient Physical Therapist – Rehab Services

About the role

The Outpatient Physical Therapist works autonomously to deliver physical therapy services to patients referred by providers or directly seeking care. The role involves evaluating patients, creating and adjusting treatment plans, monitoring progress toward goals, and contributing to program development to meet community needs.

Key responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment programs.
• Assess patient response to therapy, modify plans as needed, and document all care according to standards.
• Maintain accurate patient records and ensure compliance with documentation policies.
• Achieve productivity targets related to access to care, patient experience, and quality metrics.
• Assist in the development and enhancement of outpatient rehabilitation programs.
• Adapt communication and care approaches to respect patient privacy and diverse needs.
• Perform additional duties as assigned to support the department.

Required profile

• Current physical therapy license in the state of practice or eligibility for licensure.
• Basic Life Support (BLS) certification from the American Heart Association.
• Proficiency with computers, software, and web‑based applications.
• Effective verbal and written communication skills.
• Strong judgment, decision‑making, and interpersonal abilities.
• Flexibility to adjust schedules based on patient demand.
