Licensed Practical Nurse – Primary Care – Lake Terrace (Full‑Time)

About the role

This position provides direct patient care, including IV therapy, for assigned patients in a primary‑care setting at Lake Terrace. The LPN works under the direction of a registered nurse, collaborates with the multidisciplinary team, and assists ancillary nursing staff.

Key responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Deliver care addressing physical, psychosocial, educational, and safety needs of patients.
• Communicate professionally with patients, families, and team members to ensure a quality experience.
• Adapt to rapid changes on the unit and support fast‑paced workflow with minimal supervision.
• Maintain current clinical knowledge, credentials, and comply with federal, state, and accreditation standards.

Required profile

• High school diploma or equivalent.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Basic Life Support (BLS) certification from the American Heart Association.
• Preferred IV therapy credentials.
• Excellent organizational, time‑management, and judgment skills.
• Strong interpersonal and delegation abilities.
• Ability to handle confidential information.
