Licensed Practical Nurse (LPN) – Family Practice

About the role

Evergreen Family Medicine is seeking a Licensed Practical Nurse (LPN) to join its fast‑paced family practice clinic in Roseburg. The LPN will provide direct patient care, support providers, and help maintain a safe, efficient clinical environment.

Key responsibilities

• Obtain vital signs, medical history and medication information, and perform triage assessments.
• Administer medications, injections, and intravenous therapy under provider supervision.
• Assist with procedures such as pap exams, laceration repairs, wound care, catheter placement, and splinting.
• Perform in‑house lab tests (e.g., strep, urine dipsticks) and operate equipment like EKG, AED, ultrasound and bladder scanner.
• Document patient information accurately in the EHR and communicate findings to providers.

Required profile

• Graduated from an accredited nursing program with a valid LPN license.
• Current BLS certification.
• 2–5 years of experience in a medical setting, preferably family practice.
• Ability to work evenings and weekends.

Required skills

• Phlebotomy and intravenous therapy.
• Operation of EKG, AED, ZOLL, ultrasound and bladder‑scanner devices.
• Medication administration and wound care.
• Basic lab testing (strep, mono, urine HCG, UA).
