Licensed Master Social Worker – Transplant

About the role

The Transplant Licensed Social Worker joins the solid organ transplant multidisciplinary team at UAMS. This position provides comprehensive psychosocial support to transplant patients and their families throughout the transplant continuum. The role ensures compliance with state and federal regulations governing social work practice in a medical setting.

Key responsibilities

• Assess psychological, financial, and discharge planning needs of patients across all age groups, from neonatal to geriatric.
• Provide counseling, education, and referrals to community resources and support services.
• Collaborate with physicians, nurses, and other team members to coordinate care plans and ensure smooth transitions from hospital to home.

Required profile

• Master’s degree in Social Work and Arkansas LMSW licensure by the date of hire.
• Experience in case management, preferably in a medical or transplant setting.
• Certification such as Certified Case Manager (CCM) or American Case Management (ACM) is preferred.
