About the role
The Hospital Case Expeditor supports the transition of individuals with intellectual or developmental disabilities from state hospitals or intensive treatment settings to community-based living arrangements. Working under the Office of Crisis and Transitions, the role involves coordinating placement, information flow, and support services to ensure timely and successful community integration.
Key responsibilities
- Reduce average length of stay by managing transition activities from start to community placement.
- Assemble and lead a transition planning team for each case to identify supports, services, equipment, and supervision needs.
- Follow the Office of Transitions Guide and related manuals throughout each transition.
- Maintain an up‑to‑date inventory of community resources within the catchment area.
- Develop and sustain working relationships with internal and external stakeholders.
Required profile
- Bachelor’s degree in a related field from an accredited institution.
- At least four years of relevant professional experience, including one year in a lead, team‑leader, or supervisory role.
- Experience managing the movement of populations from secure or intensive treatment settings to less restrictive environments.
- Knowledge of Georgia’s service system for behavioral health and developmental disabilities.
- Experience working with adults who have intellectual/developmental disabilities and co‑occurring diagnoses.
