Geriatric Case Manager

About the role

Finger Lakes Community Health (FLCH) is hiring a Geriatric Case Manager to provide client-centered care planning and support for its patients aged 60 and older. The role helps patients navigate healthcare and social service systems, reduces barriers to care and promotes better health, safety and independence. It is based at the Geneva Community Health Center but involves travel to surrounding counties.

Key responsibilities

• Identify eligible patients through referrals, EHR review and morning huddles, then assess their socio-economic factors and set individualized health and social goals.
• Help patients access healthcare and social services, coordinate transportation to appointments and complete annual fall-risk and health-related social risk assessments.
• Share the care plan with providers, care managers and partners, review care documentation for gaps and refer patients to community resources such as Social Services.
• Document case details, services and referrals in the electronic health record, and reassess the care plan annually or after major life changes.

Required profile

• High school diploma or equivalent; associate or bachelor's degree in social work, public health, human services or a related field preferred.
• 2+ years in healthcare or customer service preferred.
• Valid government-issued photo ID, a valid New York State driver's license and current NY auto insurance with $100,000/$300,000/$100,000 limits.
• Ability to work in a health center, in community outreach and remotely, during standard business hours with occasional extended shifts.

Required skills

• Working knowledge of community health principles in outreach and support programs.
• Clear vision and hearing, able to type frequently and lift up to 15 pounds occasionally.
