Full‑Time Nurse Practitioner – Dementia Care

About the role

Tembo Health is seeking its first full‑time Nurse Practitioner to lead virtual dementia assessments and support families throughout the diagnostic journey. You will work closely with the Head of Medical and care navigation team to refine workflows and deliver patient‑centered care.

Key responsibilities

• Conduct virtual assessments for patients with suspected dementia, including baseline cognitive testing and ordering laboratory work as needed.
• Perform medication reviews, recommending reductions of high‑risk drugs and initiating appropriate dementia‑related therapies.
• Develop comprehensive care plans in collaboration with care navigators.
• Coordinate care with primary care providers, specialists, and external care teams to support patients and caregivers.
• Address urgent clinical needs and other geriatric practice requirements as the program evolves.

Required profile

• Licensed Nurse Practitioner with a passion for innovative dementia care.
• Experience conducting virtual patient assessments.
• Ability to collaborate with multidisciplinary teams.

Required skills

• Athena Health electronic medical record system.
