Field Nurse Practitioner – In-Home Assessments (PRN)

About the role

Advantmed is seeking a licensed Nurse Practitioner to perform structured in‑home wellness and risk‑adjustment assessments for Medicare members and other at‑risk populations in Boone County, Missouri. This PRN, 1099 position offers flexible weekday scheduling, paid mileage and a per‑visit compensation of approximately $100.

Key responsibilities

• Conduct in‑home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, physical exams, vitals, medication reviews and specific screenings (Hemoglobin A1C, Spirometry, Kidney Health Evaluation, Diabetic Retinal Exam, FIT).
• Educate members on preventive care and chronic disease management.
• Document all findings in the EMR to support value‑based care and risk adjustment.

Required profile

• Active, unencumbered Nurse Practitioner license in Missouri (or ability to obtain additional licensure).
• 3+ years of patient‑care experience preferred.
• Comfortable working independently in the field and responsive to scheduling.
• Availability of approximately 30 hours per month on a consistent basis.

Required skills

• Proficiency with electronic medical record (EMR) systems.
