Case Manager – Youth Services

About the role

The Full‑time Case Manager will support youth participating in the YHDPA Program of Comprehensive Youth Services. The role requires a trauma‑informed, positive youth development approach and involves managing intake, assessments, and ongoing case coordination.

Key responsibilities

• Assist with intake assessments, case management paperwork, and timely documentation.
• Oversee client educational referrals and coordinate housing assistance.
• Answer agency crisis, information, and referral calls.
• Participate in intrastaff and monthly staff meetings.
• Provide individual and family case management, linking clients to community resources.
• Complete client care monitoring at least once a month and manage after‑care follow‑ups.
• Compile data and narrative descriptions for quarterly, semi‑annual, and annual reports.
• Enter and report data in RHYMIS and HMIS systems.
• Complete required annual training hours and comply with licensing requirements.
• Participate in community coalitions and networks as needed.

Required profile

• Bachelor's degree (or active pursuit) in criminal justice, psychology, social work, or guidance and counseling.
• Valid driver’s license with a clean driving record and ability to pass MDHHS background checks.
• Experience or knowledge of youth development, advocacy, and case management is desired.
