Cardiology Patient Navigator (Coordinator)

About the role

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team. The role focuses on patient communication, scheduling, care coordination, and clinical documentation for a high‑volume remote patient caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems, and conduct telephonic or remote outreach to reinforce care instructions.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows, coordinating with the clinical team for a caseload of 150+ patients.
• Maintain accurate clinical documentation in EMR/EHR, ensure HIPAA compliance, and support billing and insurance coordination.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent) with 3+ years experience in healthcare, telehealth, or medical office settings.
• Strong patient‑centered communication skills, bilingual English/Spanish proficiency (B2+), and ability to work U.S. business hours (CST).

Required skills

• Proficiency in remote patient monitoring platforms.
