Cardiology Patient Navigator (Coordinator)

About the role

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. The ideal candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high‑volume patient caseload across multiple priorities.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology care pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
• Coordinate with the clinical team to ensure continuity of care and escalate clinical concerns as needed.
• Maintain accurate clinical documentation in EMR/EHR systems and ensure HIPAA compliance.
• Support billing documentation and insurance coordination when required.
