About the role
Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy U.S.-based cardiology care team. The role focuses on patient communication, scheduling, care coordination, and clinical documentation, working remotely during U.S. Central business hours. Ideal candidates have a clinical background and experience in telephonic or remote care settings.
Key responsibilities
- Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
- Conduct telephonic or remote outreach to reinforce care instructions and coordinate next steps.
- Track testing schedules, appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
- Coordinate with the clinical team to ensure continuity of care and escalate clinical concerns as needed.
- Maintain accurate clinical documentation, ensure HIPAA compliance, and support billing and insurance coordination.
Required profile
- Patient‑first attitude with strong phone communication skills.
- Comfortable managing a high‑volume, remote environment with 150+ patients.
- Self‑directed, proactive communicator who adapts to evolving tools and workflows.
- Bilingual English/Spanish proficiency (B2+).
Required skills
- Experience with EMR/EHR systems.
- Familiarity with remote patient monitoring platforms.
- Knowledge of medical billing and prior authorization processes.
