Ambulatory Clinical Pharmacist – Pulmonology & Asthma

About the role

The Ambulatory Clinical Pharmacist will manage medication therapy for patients with pulmonary and asthma conditions in an outpatient setting, ensuring safe, effective, and cost‑efficient care while supporting education of residents and other health‑care professionals.

Key responsibilities

• Perform medication reconciliation, monitoring, and patient education for assigned ambulatory patients.
• Provide pharmacokinetic consultations and evaluate drug selection, dosing, and patient response.
• Develop and update clinical pathways, best‑practice guidelines, order sets, and formularies for the pulmonology population.
• Educate pharmacy and medical residents, students, staff, and other health‑care providers.
• Support transitions of care and overall patient outcomes beyond the hospital visit.

Required profile

• Bachelor of Science in Pharmacy (BS Pharm) and a current state pharmacy license.
• Completion of a PGY1 Pharmacy Practice Residency (or three years related specialty experience).
• Preferred: Doctor of Pharmacy (PharmD) and completion of a PGY2 Pharmacy Residency.

Required skills

• Proficiency with Windows‑style computer applications.
• Strong computer data entry and information retrieval skills.
