Ambulatory Care Clinical Pharmacist

About the role

Take your next step to join a collaborative practice model within Kingman Regional Medical Center’s Disease Management Clinic as an Ambulatory Care Clinical Pharmacist. The position is full‑time, rotating shifts, and focuses on disease‑state medication management in an outpatient setting.

Key responsibilities

• Provide medication management for disease states such as hepatitis C, anticoagulation, diabetes (including insulin pump therapy), hypertension, and dyslipidemia under a Collaborative Practice Agreement.
• Utilize delegated prescriptive authority and laboratory ordering to work at the top of the pharmacist scope of practice.
• Collaborate with primary care, endocrinology, oncology, cardiology, hepatology, and other community practitioners as part of a multidisciplinary team.

Required profile

• Doctor of Pharmacy (Pharm.D.) degree from an ACPE‑accredited school.
• Arizona pharmacy license (must be obtained within 90 days of hire).
• Preferred but not required: completion of PGY‑1/PGY‑2 residency or board certification.
• Physical ability to sit, stand, walk, lift 25‑75 lb and distinguish colors as described.
