Advanced Registered Nurse Practitioner (ARNP) – Part‑time Remote

About the role

Nolia Health is seeking a California‑licensed Advanced Registered Nurse Practitioner to join its virtual care team on a part‑time contract basis. You will deliver high‑quality telehealth services to seniors with chronic conditions, beginning with dementia, and support their family caregivers.

Key responsibilities

• Navigate and manage complex chronic comorbidities for senior patients.
• Create, implement, and adjust personalized treatment and care plans in collaboration with a transdisciplinary team.
• Monitor patient progress, conduct screenings for social determinants of health and behavioral needs, and update medical records.
• Provide clinical education to patients, families, and staff via virtual care conferences.
• Collaborate with MDs, neuropsychologists, social workers, and care navigators to ensure coordinated, holistic care.
• Participate in quality‑improvement initiatives and contribute to the strategic direction of Nolia’s care services.

Required profile

• California‑licensed Nurse Practitioner with experience in geriatrics or chronic disease management.
• Proven ability to deliver care remotely through telehealth platforms.
• Strong interpersonal, leadership, and problem‑solving abilities.
• Commitment to patient‑centered care and continuous learning.
