Adult Guardianship Case Manager 2

About the role

The Georgia Department of Human Services is seeking an Adult Guardianship Case Manager to provide case management services for adults placed under public guardianship. The position serves Columbus and nine surrounding counties, coordinating care and support for vulnerable adults.

Key responsibilities

• Provide guardianship case management in accordance with Georgia guardianship law, probate court orders, and department policies.
• Develop, implement, and monitor individualized care plans that address support, health, education, and welfare needs.
• Practice surrogate decision‑making that respects the client’s expressed wishes and seeks the least restrictive alternatives.
• Advocate on behalf of the client and collaborate with trustees, conservators, and other stakeholders.
• Travel regularly throughout the assigned district to meet clients and service providers.

Required profile

• Ability to deliver case management services under state guardianship statutes.
• Experience working with vulnerable adult populations.
• Willingness to travel across multiple counties as needed.
• Strong communication and coordination skills.
