Account Manager, ACOs & Medicare Supplement (MediGap) Health Plans

About the role

The Account Manager for ACOs & Medicare Supplement (MediGap) Health Plans will partner with ACOs and MediGap carriers to drive enrollment and program adoption of Ceresti Health’s GUIDE Model for dementia care. This remote, full‑time position sits at the intersection of performance management, physician education, and carrier relationship development.

Key responsibilities

• Serve as the primary liaison for ACOs and MediGap carriers, translating partnership goals into enrollment performance.
• Develop and nurture trust with physician stakeholders, operational teams, and carrier product/actuarial groups.
• Manage account health, monitor ROI, and report measurable outcomes to partners.
• Coordinate integration of GUIDE Model workflows with carrier member services.
• Identify and address compliance and CMS requirements across both partner types.

Required profile

• Proven experience managing complex healthcare accounts, preferably with ACOs or Medicare Supplement plans.
• Strong understanding of Medicare Advantage, CMS guidelines, and value‑based care models.
• Excellent relationship‑building and communication skills with clinical and commercial stakeholders.
• Ability to work independently in a remote environment and manage multiple priorities.
